Please review carefully the guidelines for creating and hosting a Fundraiser (the "Fundraiser" or "Fundraisers") on behalf of St. Michael's Hospital Foundation ("SMHF").
By clicking on the AGREE button you (the "Organizer") are confirming that you have read, fully understand and willingly agree to abide by the terms and conditions outlined below.

FUNDRAISER POLICY GUIDELINES
Once you complete your registration, you can begin fundraising immediately. Please note, that all Fundraisers are reviewed by an SMHF representative.
SMHF reserves the right to reject any Fundraiser that is considered incompatible with SMHF's values or image or which may be deemed deleterious to our brand.
No agreements or contracts must be entered into using SMHF's name, nor should the Organizer attempt to obligate SMHF in any way without prior written approval from SMHF.
The Fundraiser must be carried out in compliance with all applicable laws, rules and regulations of any applicable governing body.
The Organizer may respond to queries relevant to the Fundraiser, but any inquiries relating to St. Michael's Hospital or SMHF must be forwarded to SMHF.
DIRECTED DONATIONS
St. Michael's Hospital Foundation uses every dollar raised as efficiently as possible to ensure that the St. Michael's Hospital has the research, treatment, equipment and infrastructure it needs to remain at the cutting-edge of medicine.
For all directed donations, a foundation contribution of 10% will be deducted to cover general fundraising and administrative expenses.
INSURANCE
SMHF cannot provide insurance coverage for Fundraisers. The Organizer confirms that if required and at its own expense, adequate insurance will be obtained for the Fundraiser.
EXPENSES
Expenses related to your Fundraiser are at your own cost and cannot be deducted from the donations collected through your Fundraiser.
TAX RECEIPT
Issuance of tax receipts must be in compliance with Canada Revenue Agency's guidelines. Tax receipts will be issued automatically for donations of $20 and above made through SMHF's online donation platform. Tax receipts for offline donations will be issued for donations of $20 and above. For all other types of donations/gifts, including but not limited to in-kind gifts and offline donation submissions, the Organizer must have pre-approval from an SMHF representative before promising any income tax receipts to patrons/supporters of the Fundraiser.
PRIVACY

As an Organizer you may have access to information relating to donors that is confidential and private in nature. You will, at all times, ensure the protection and confidentiality of all donor information, particularly any personal information collected in the course of soliciting funds. Under no circumstances may donor confidential or personal information be shared with any other individual or third party; it remains the property of SMHF and must be returned to SMHF.
WAIVER
I acknowledge that I understand the intent thereof, and I hereby agree and absolve and hold harmless St. Michael's Hospital Foundation, singly or collectively, from and against any blame and liability for any fraudulent acts of statements made by the Organizer, injury, misadventure sustained as a result of participation in the Fundraiser for SMHF or any activities associated wherewith.
I ACKNOWLEDGE HAVING READ, UNDERSTOOD AND AGREED TO THE ABOVE WAIVER, RELEASE AND INDEMNITY.
