My target 21 kms
I'm sweating it out for St. Michael's Hospital!
This year I am running the Oakville 21.1k to raise funds for the Navigator Program at Unity Health Toronto (St Michael’s Hospital).
People experiencing homelessness suffer from poor outcomes after hospitalization due to barriers to care, sub optimal transitions of care, and intersecting health and social burdens.
The Navigator Program is a critical case management program that was developed to help homeless patients with their post discharge needs, and to link them with community based health and social services.
Please help me cross the finish line with a real sense of purpose, and hit my fundraising goal!
